What Should You Do Immediately After a Suspected Concussion?

When in doubt, sit them out.
If concussion is suspected during sports or physical activity, the individual should be removed from play immediately.
They should not return to play on the same day.

Watch for concerning changes such as worsening headache, repeated vomiting, seizure activity, or other significant deterioration, and seek urgent medical attention when appropriate.

Why “Cocooning” After Concussion Has Changed
For years, concussion patients were commonly told to lie in a dark room, avoid screens, stop exercising, minimize mental stimulation, and wait until every symptom disappeared.
This approach became known as “cocooning.”
Modern concussion management has moved away from prolonged total inactivity.
Instead, an initial period of approximately 24–48 hours of relative physical and cognitive rest may be followed by a gradual, symptom-limited return to appropriate activity under professional guidance.
The objective isn’t to push through severe symptoms.
It is to gradually reintroduce tolerable activity as recovery progresses rather than unnecessarily isolating the brain from all stimulation for an extended period.
Return to Learn Before Return to Play
For children, teenagers, and student athletes, returning to the classroom is an important part of concussion recovery.
The guiding principle is:
Return to Learn should precede Return to Play.
A student who still cannot tolerate reading, classroom noise, visual stimulation, or sustained concentration may not yet be ready for unrestricted athletic competition.
Depending on symptoms, temporary academic accommodations may be needed while the brain recovers.
Recovery should be individualized rather than based solely on a predetermined number of days.
How Does Return to Play Work?
Returning to sport should be progressive rather than immediate.
A staged process may begin with symptom-limited activities such as walking or light cycling before progressing toward more demanding exercise.
Later stages may include sport-specific non-contact drills, followed by full-contact practice after appropriate medical clearance and, ultimately, a return to competition.
If symptoms return as activity increases, this is important information.
It may indicate that the athlete needs to reduce the level of activity and receive further assessment before progressing.
The central principle is that returning to sport should be based on neurological recovery—not pressure from a competition schedule.
What Happens When Concussion Symptoms Don’t Go Away?
Many people recover within the expected timeframe, but some continue experiencing symptoms.
Persistent problems may include:
- Dizziness
- Balance difficulties
- Neck pain
- Headaches
- Visual disturbances
- Brain fog
- Difficulty concentrating
- Fatigue
- Exercise intolerance
